This Sample Safety Walk form is an example of how Managers/Leaders can record observations following a safety walk of the workplace. A safety walk or safety observation is a proactive process used in workplaces, especially in industries such as construction, manufacturing, and logistics, to identify and address potential safety hazards before they lead to incidents. Common elements observed during a safety walk include use of PPE (Personal Protective Equipment), housekeeping and cleanliness, machine guarding and equipment condition, ergonomics and body positioning, signage, lighting, and emergency access.
