The term “parental leave” is used collectively throughout the National Employment Standards (NES) and Fair Work Act 2009 to cover maternity, paternity and adoption leave and covers leave which is related to the birth or adoption of a child. This form is for employees to complete to notify or update the details of any parental leave that they intend to take to assist an employer to plan for the absence.
