A Performance Improvement Plan (PIP) is a formal, structured tool used when ongoing performance gaps have not been resolved through informal feedback and monitoring. This form will support fair, evidence‑based and constructive conversations by clearly documenting performance concerns, required standards, supports and review processes, with the aim of achieving sustainable improvement rather than serving as a disciplinary pathway.
